Are You A Good Candidate for Bariatric Surgery?
Below is a brief outline of the steps involved in applying to the Fletcher Allen Bariatric Surgery program.
Step One: Complete and Return a Health Questionnaire
Request a Health Questionnaire through our online request form or call to request one be mailed to you.

Step Two: Attend an Introduction Session
Once the questionnaire is received and reviewed by our staff, you will be contacted and invited to attend an Introduction Session held at the Bariatric Clinic in Williston. This 2-hour session will explain our current surgical options and provide you with information about our program requirements.
Step Three: Complete a Psychological Evaluation
Our team psychologist will evaluate you during a private office visit to assess readiness for surgery and to identify contraindications for surgery.
Step Four: Meet with a Surgeon, Dietitian and Physician Assistant
For the next several months, you will have monthly visits to the Bariatric Clinic. Further medical testing may be necessary to ensure your ability to undergo a surgical procedure. The dietitian will set an individual weight goal for you and follow your progress towards that goal. You will be asked to attend a pre-op nutrition class and a behavioral skills class prior to surgery as well as one support group meeting.
