Inpatient Acute Rehabilitation Referral Information
To be eligible for admission all patients must:
- Require daily access to a physiatrist, who will oversee the patient's medical needs and functional plan of care.
- Require occupational and physical therapy.
- Be able to tolerate a relatively intense therapy program, consisting of at least three hours of therapy per day, five days a week.
- Have the ability to achieve rehabilitation goals in a reasonable time period.
Referrals may be made by a physician, nurse, therapist, social worker, case manager or discharge planner. A Rehabilitation Admissions Coordinator will perform a pre-admission assessment including information from the patient's medical record, discussion with the patient's care team, and an interview with the patient and family. If necessary, the coordinator will contact the referring physician. The referrer is generally informed of an admission decision within one business day.
