South Burlington Family Practice is located approximately 2 miles east of Burlington in South Burlington, Vermont.

The practice opened in December 1997, and has grown to over 800 patient visits per month.

The providers care for patients of all ages, providing a broad range of services including adult and child preventative medicine, obstetric care, and acute care needs.

As a nationally certified patient-centered medical home, we use a team approach to work with you to prevent illness.
