Milton Family Practice is 14 miles north of Burlington in Milton, Vermont, and is the model ambulatory practice site. The practice was established in 1973 and has grown to well over 2000 patient visits per month. Residents and faculty working here provide a broad range of services including obstetrics and home visits.

We are nationally certified patient-centered medical home and prevent illness by working with you to keep you as healthy as possible.
