Background

Colchester Family Practice is located approximately four miles northwest of Burlington in Colchester, Vermont. It is a 7,000 square foot office facility near the Colchester Town Office.

The patient care team, are known as a a nationally certified patient-centered medical home, includes doctors, nurse practitioners, physician assistants, a counselor (licensed social worker), and support staff. The group provides care from newborns through to geriatrics and includes obstetrics.
