Berlin Family Health is located at Central Vermont Medical Center in Berlin. The practice was established in 1976, joined Fletcher Allen Health Care in November 1997, and has grown to over 1900 visits per month.
Our clinic is a nationally certified patient-centered medical home. We use a team approach to work with you to prevent illness and keep you as healthy as possible.
The five Board-Certified family physicians, a nurse practitioner, and a physician assistant provide a wide range of care to patients of all ages, including nursing home and home visits. Two behavioral health providers are on site to provide care as well.
