Palliative Care Mentorship Program
Through our mentorship program, community providers can spend a day or longer with the Fletcher Allen Palliative Care Service. It gives providers in rural areas the opportunity to observe and learn from a tertiary care palliative care team. The goal is to help clinicians and rural community hospitals and hospice programs develop their expertise in palliative care and end-of-life care. Mentorship Objectives
- To develop a longer-term relationship with rural community hospitals and providers.
- To enhance clinical knowledge and skills in the area of palliative care and hospice medicine.
- To foster an ongoing palliative care discussion group among rural hospitals focused on their unique needs.
- To explore opportunities for ongoing quality improvement and research.
Who Should Attend the Palliative Care Mentorship Program?
Health care professionals in Vermont and northern New York who want to improve their knowledge and skills in palliative care and hospice medicine should attend, including:
- Physicians
- Nurses
- Hospice care providers
- Physician assistants
- Nurse practitioners
- Chaplins
- Social workers

Program Schedule and Curriculum
Mentorships take place Monday through Wednesday from September to May. A maximum of two clinicians are accepted to the program at any one time.
The curriculum includes:
- Individual needs assessment
- Palliative Care morning report; reviewing all active patients
- Educational module on management of pain and non-pain symptoms; how to conduct a palliative care consultation
- Patient rounds
- Noon conference (clinical case conferences)
- Learning modules
- Communication skills and advanced care planning
