How to Request Your Medical Records

You can request paper copies of your medical record or radiology images from the Fletcher Allen Health Information Office by following the instructions below.

To Request Your Medical Record

If you are requesting copies of your medical record, please print out the authorization to release patient information form and fax, mail or bring it to your physician's office. The contact information for the physician's office will appear on the document.

To Request Radiology Images

If you are requesting copies of your radiology images, such as x-rays, CT scans or MRI films, please contact the Imaging Library at 802-847-2354.

To Access Your Medical Record Online

Portions of your medical record are available through MyHealth Online, a new feature of our electronic health record. You also may print the medical record information available on MyHealth Online. Learn more about MyHealth Online or request an account.

If you have any questions, please contact the Fletcher Allen Health Information Office (Medical Records Office), Monday-Friday, 8:00 am - 4:30 pm at 802-847-2846.

If you have an emergent issue arising during normal business hours, call your physician's office.

Please Note

If you request us to send records to another provider or facility there will be no charge. If your records are going to be sent to somebody other than a provider or facility the first 1 to 25 pages will be copied by the copy service and mailed with a bill. If the records are over 25 pages you will receive a pre-bill from the copy service that will need to be paid before the reproduction of the records. A flat rate of $5.00 for the first 1 to 10 pages will be charged. If over 10 pages, the charge is $0.50 cents per page and $1.00 per page for microfilmed records after the first 10 pages.

Once your medical record request is received it takes approximately three to four weeks for processing.

DISCLAIMER: Incomplete or preliminary information from the medical record will only be released in urgent or emergent situations and only for patient care. For all other requests, the medical records must be in a completed status before any information will be released. Any exceptions to this rule need to be authorized by a member of the management in the Health Information Management Department (HIM) or Case Management.
