Referral System
There are several ways to contact our clinic to schedule an appointment for your patient. You can call 802-847-CARD (2273) or (toll free) 866-283-6892. You can send an email to cardschedule@vtmednet.org. For questions related to pacemakers or implanted defibrillators you can send an email to pacermd@vtmednet.org. Information can be faxed to 802-847-1422, attention Dawn.
You can expect that your patient will be seen by an electrophysiologist within two weeks of contacting our schedulers. If you would like a patient seen sooner please call Dawn Fragola, EP Outpatient nurse coordinator at 802-847-6209. If a patient asks to speak with someone they can call Dawn at 802-847-9675.
Our outpatient clinic is located at 62 Tilley Drive in South Burlington. We also see patients at clinics located in Malone, Massena and Canton-Potsdam New York. Patients can call 802-847-4600 (toll free: 800-358-1144, ext 74600) for addresses and directions to all of these clinics.
Obtaining Inpatient Arrhythmia Service Consults:
In-patient arrhythmia service consults at Fletcher Allen can be obtained by paging the “EP Consult fellow” through PAS (7-2700) 5 days a week, 7 am to 5 pm. After-hours and weekends, the on-call Cardiology fellow will perform initial evaluations in conjunction with the on-call Cardiology Attending.
